Diabetes Watch

Diabetes Watch is a planned programme of care for those patients with a diagnosis of diabetes melitus.

The aims of the programme is to maintain the patient’s health and prevent the complications of diabetes.

The services provided include regular check ups to monitor the patient’s risk factors for heart disease and diabetic medications as well as screening for early signs of diabetic complications. It is  provided in-practice by our team of nurses, doctors, dietician, podiatrist and eye specialist.

Most services are provided for free to those enrolled in the programme.

Heart Watch

Heart Watch is a national programme for the secondary prevention of ischemic heart disease.

The aim is to maintain the heart health and prevent future problems in those who have experienced a heart attack, or who have had a stent or bi-pass procedure.

The services provided include regular check ups to monitor the patient’s risk factors for heart disease and cardiac medications. This planned programme of care is provided in-practice by our team of nurses, doctors and dietician.

Most services are provided for free to those enrolled in the programme.

Chronic Disease Management Programme

The roll-out of a new Chronic Disease Management Programme (CMD) for  has started.

The first phase of the Programme is targeting GMS/GPVisit Card patients, aged over 75 years, with the following chronic diseases:

  • Diabetes Type 2
  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Cardiovascular Disease including:
    • Heart Failure
    • Ischaemic Heart Disease
    • Cerebrovascular Disease (Stroke/ Transient Ischemic Attack (TIA))
    • Atrial Fibrillation

As part of the structured CDM programme, patients will provide twice-yearly scheduled reviews with the Practice Nurse and GP. Planned investigations such as blood tests will be carried out at the practice nurse visit, prior to the GP consultation. These reviews will include patient education, preventative care, medication review, physical examination, investigations and an individual care plan.
